Chapters Transcript SureStep™ Intermittent Catheter Introduction. catheter associated urinary tract infections, also known as coties, are associated with increased morbidity, mortality, and costs. Guidelines are in place to reduce coy risk factors and have led to extensive focus being placed on improving aseptic insertion practices. As a result, hospitals have made considerable improvements in Foley catheter insertions. However, over 40% of urinary catheter insertions are performed with intermittent catheters that require the same aseptic insertion technique as a Foley catheter. Differences between Foley and intermittent catheter trays increase practice variation and the risk for non-septic insertions. The Sure Step Intermittent catheter tray is designed to help hospitals standardize urinary catheter insertions to reduce practice variation and encourage proper aseptic technique, no matter which type of catheter is used. Proper technique in procedural setup. Proper technique for urinary catheter insertion. Perform hand hygiene immediately before and after insertion. Insert urinary catheters using aseptic technique and sterile equipment. Use the smallest catheter possible consistent with good drainage and document the indications for catheter insertion, date and time of catheter insertion, and the individual who inserted the catheter. Prior to starting the procedure, obtain all of your necessary supplies, clean gloves, and the sure step intermittent catheter kit. Perry care and insertion. Wash hands and don clean gloves. Explain the procedure to the patient and open the perrya kit. Use the provided packet of wipes to cleanse the patient's periurethral area. Remove gloves and perform hand hygiene with provided alcohol hand sanitizer gel. Using proper aseptic technique, open the CSR wrap. Da sterile gloves. Place under pad beneath the patient with the plastic or shiny side down. Position fenestrated drape on the patient. Saturate 3 foam swab sticks in Povidone iodine. Lubricate the catheter. Prepare the patient with the 3 foam swab sticks saturated in Povidone iodine. Use your non-dominant hand for the genitalia and your dominant hand for the swab sticks. Note Use each swabs stick for one swipe only. Female patient with a downward stroke cleanse the right labia minora and discard the swab. Do the same for the left labia minora. With the last swabs stick, cleanse the middle area between the labia minora. Male patient cleanse the penis in a circular motion starting at the urethral meatus and working outward. Proceed with catheterization using your dominant hand. When the catheter tip has entered the bladder, urine will be visible in the drainage tube. Document the procedure according to your hospital protocol. Barred easy lock sampling port indicated by the blue stem in the port accepts lurelock or slip tip collection devices. BD vacutainer urine products can be used for collection, storage, and transport of urine specimens from a urinary catheter. Swab the surface of the barred easy lock sampling port with an antiseptic such as the sight scrub IPA device, which is an effective disinfecting agent for lure hubs. Using aseptic technique, position the collection device in the center of the sampling port. Press the device firmly and twist gently to access the sampling port. If using the BD vacutainer lurelock access device, collect urine into the BD vacutainer tubes per your hospital protocol. If using a syringe, slowly aspirate the urine sample into the syringe and transfer the sample to a collection cup or follow your hospital protocol for sample transfers. After the sample is obtained, discard the collection device according to your hospital's protocol. Follow your hospital's protocol for specimen labeling and transport. Indications intermittent catheters are intended for use in the drainage and or collection and or measurement of urine. Cautions Federal USA law restricts these devices to sale by or on the order of a physician. Sterile unless package is opened or damaged except for any individually packaged components within the tray which are not labeled as sterile. These components are not terminally sterilized, single use only. Do not re-sterilize for urological use only. Warning on catheter. Do not use ointments or lubricants having a petroleum base. They will damage the catheter. Warning, after use, this product may be a potential biohazard. Handle and dispose of in accordance with accepted medical practices and applicable local, state, and federal laws and regulations. Visually inspect the product for any imperfections or surface deterioration prior to use. If package is opened or if any imperfection or surface deterioration is observed, do not use.
